The five-step service model
Every service we provide — for a direct client or an authorizing agency — moves through the same five steps, applied with the same standard of safety, documentation, and accountability. Safety always comes before routine assessment and longer-term planning.
Accept the referral
Accept the referral or intake request; capture referral source, contact information, reason for referral, funding source, and permission to follow up.
Confirm safety, first
Determine whether the client is presently safe. When necessary, immediately coordinate a safe temporary location before addressing any other concern.
Understand the full picture
Identify urgent needs, goals, barriers, benefits status, current services, preferred contact method, strengths, and available resources.
Coordinate across agencies
Work across appropriate agencies and organizations, make warm referrals, help resolve barriers, and document every contact and decision.
Stay until it’s resolved
Agree on next actions, coach the client through those actions, monitor progress, update the plan, and document outcomes.
Inside a coaching session
Every coaching contact, however short, follows the same flow — so nothing that matters gets missed.
- ✓Check safety, urgent changes, and immediate barriers
- ✓Review the last agreed actions and what occurred
- ✓Recognize progress, strengths, and completed steps
- ✓Choose the most important current priority
- ✓Break the priority into specific, manageable actions
- ✓Confirm who will do what, and by when
- ✓Schedule the next contact and document the session
Coaching standard
The client remains the decision-maker. Network32 provides structure, information, encouragement, accountability, and practical assistance — while respecting the client’s choices, pace, abilities, culture, and circumstances.
Six elements, every plan
| Element | What to record |
|---|---|
| Goal | The result the client wants to achieve. |
| Current situation | Relevant facts, strengths, needs, and barriers. |
| Next action | A specific step that can be completed or measured. |
| Responsibility | The client, Network32, an agency, or another authorized person. |
| Target date | When the action should occur or be reviewed. |
| Progress | Completed, in progress, waiting, changed, or stopped. |
What advocacy looks like here
- ✓Help identify the correct benefit, service, office, or process
- ✓Prepare the client for calls, appointments, applications, or documents
- ✓With permission, communicate with agencies to clarify status or next steps
- ✓Help organize facts and questions so the request is clear and complete
- ✓Track responses, deadlines, decisions, and follow-up options
The warm referral standard
A warm referral means more than a name or a phone number. With the client’s permission, we help make the connection, confirm the resource is appropriate and available, share necessary background, set expectations, and follow up to confirm contact or placement happened.
Every case has a next action, an owner, and a date
Follow-up continues at a level appropriate to the client’s needs, service authorization, risk, progress, and responsiveness — tracked across five outcome areas.
| Outcome area | Examples of progress |
|---|---|
| Safety | Safe temporary location, crisis connection, or reduced immediate risk. |
| Access | Completed application, scheduled appointment, active referral, or benefit connection. |
| Stability | Housing progress, reliable services, improved follow-through, or reduced barriers. |
| Independence | Client completes actions, uses resources, communicates needs, and manages next steps. |
| Continuity | Client remains connected to appropriate support and knows the next action. |
The scope of our role
Network32 provides coaching, advocacy, coordination, and follow-through. These services are not presented as counseling, psychotherapy, medical treatment, legal representation, emergency response, or financial advice. When a client’s needs fall outside our role, we coordinate an appropriate referral.

Direct client services and veteran housing support both run on this same framework.
